New patients:
Please fill out the online New Patient Intake forms emailed to you before you arrive to your appointment. If you prefer to fill them out on a physical copy, arrive 15 minutes before your appointment to allow for time. Click here if you would prefer to fill out a paper copy beforehand.
Cancellation Policy:
We require 24 hours notice for all cancellations. Appointments cancelled less than 24 hours in advance will incur the following charge on the credit card you gave at the time of booking: $40 for Community Room appointments, full price of the appointment for Private treatments.
